Patient Care Advocate

Community Medical Group is seeking a personable, driven, and community-focused Patient Care Advocate to join our team. As a Patient Care Advocate, you will play a vital role in connecting seniors and community members with high-quality healthcare services. This position is responsible for building relationships within the community, educating prospective patients about Community Medical Group's healthcare model, and supporting patient enrollment and retention efforts. The ideal candidate enjoys engaging with people, developing partnerships, and making a meaningful impact on the health and well-being of the communities we serve.

Key Responsibilities

โ€ข Build and maintain strong relationships with seniors, community organizations, referral partners, and local businesses to generate new patient opportunities

โ€ข Educate prospective patients on Community Medical Group's services, value proposition, and healthcare model

โ€ข Conduct outreach activities in community settings, including senior centers, housing communities, health fairs, educational events, and local organizations

โ€ข Address patient questions, concerns, and service-related issues while providing exceptional customer service

โ€ข Collaborate with internal teams to ensure a seamless patient experience and support retention initiatives

โ€ข Maintain accurate records and reports related to outreach and membership activities

โ€ข Travel locally throughout the community to conduct outreach and relationship-building activities

โ€ข Perform additional duties as assigned

Qualifications

โ€ข High School Diploma or GED required; Bachelor's degree in Business, Marketing, Healthcare Administration, or a related field preferred

โ€ข Minimum of 2 years of sales, business development, community outreach, customer service, or healthcare-related experience required

โ€ข Experience working with senior populations, Medicare beneficiaries, or healthcare services preferred

โ€ข Proven ability to build and maintain professional relationships with community partners and referral sources

โ€ข Strong presentation, communication, and interpersonal skills

โ€ข Excellent organizational skills with the ability to manage multiple priorities in a fast-paced environment

โ€ข Proficiency with Microsoft Office applications required

โ€ข Experience utilizing CRM systems such as Salesforce preferred

โ€ข Strong problem-solving, negotiation, and relationship-building skills

โ€ข Ability to work independently while collaborating effectively with a team

โ€ข Valid Florida Driver's License and reliable transportation required

โ€ข Ability and willingness to travel locally within the community as needed

โ€ข Bilingual in English and Spanish or English and Creole preferred
