Job description

Currently seeking a Consultant Pharmacist for a hybrid position in Long Term Care. The position is part time for 24hrs/wk in Camden county. Must have NJ Board of Pharmacy license.

Our Consultant Pharmacists are professionals who are trained to share responsibility for the patient's medication regimen; they have access to the latest and best information on drug interactions, thorough knowledge of clinical outcomes, quality assurance, state and federal regulations, and health care education.
This position pays $50.00 per hour.

Responsibilities
A day in the life of a Consultant Pharmacist consultant:
  • Travel to long term healthcare facilities in your region (typically within 45 min from your home)
  • Evaluate the appropriateness and effectiveness of medication regimens. Areas of focus will include antimicrobial therapy, pain management, use of psychoactive medications.
  • Collaborate with physicians and other health care providers in managing medication therapy to ensure patient needs are met.
  • Apply specialized knowledge of the scientific and clinical use of medications; including medication action, dosing, adverse effects, and drug interactions.
  • Active role in the development and implementation of protocols, antimicrobial stewardship initiatives and appropriate drug utilization.
Staff education:
  • Provide education to the nursing staff through in-services and education material.
  • Provide input into formulary interchange program.
  • Educate healthcare team on new formulary medications, updated policies and procedures, medication safety initiatives and drug shortages.
  • Interdisciplinary relationships: Strives to develop a good working relationship with the medical and nursing staff in addition to other multidisciplinary departments by actively seeking out opportunities to improve patient care and medication management.
  • The practitioner routinely attends QAPI meetings, antibiotic stewardship meetings and psychoactive therapy meetings.
