Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to patient needs, goals, and physician direction. This role is responsible for delegating and directing therapy services, ensuring the design of functional treatment programs that facilitate effective community re-entry. The therapist also serves as an educator for patients, families, and colleagues, and performs other professional duties related to patient care and program development.
Responsibilities
  • Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans.
  • Document patient care accurately and timely in accordance with state and federal guidelines.
  • Ensure care is provided under valid physician orders and within insurance approvals.
  • Participate in student educational programs, providing guidance and evaluation as needed.
  • Create and pursue a professional development plan, engaging in continuing education and professional activities.
  • Adhere to state practice acts and maintain professional licensure in good standing.
  • Engage in performance improvement initiatives, providing feedback to enhance service quality.
Required Experience / Certifications / Licensure
  • Degree in Physical Therapy from an accredited institution.
  • Minimum of 1 year of outpatient clinical experience.
  • Current CPR certification.
  • Active state licensure for physical therapy.
  • Previous experience with Select Medical is a plus.
