We are currently seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our healthcare team in Greenville, TX. Position Overview

As an LVN on our team, you will provide high-quality nursing care to patients, assist with care coordination, and support remote monitoring initiatives to improve patient outcomes. This role requires excellent clinical skills, attention to detail, and the ability to work collaboratively within a multidisciplinary team.

Key Responsibilities
  • Provide direct nursing care to patients in accordance with established protocols and standards.
  • Assist with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, including patient education and monitoring.
