Job description

Patient Bridge Health is seeking dedicated and compassionate Medical Assistants to support our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role focuses on helping patients manage chronic conditions between office visits by providing education, monitoring health data, and maintaining regular communication with patients and care teams.
We are especially seeking bilingual Medical Assistants who can help us better serve diverse patient populations.
Bilingual candidates are strongly encouraged to apply, particularly those fluent in:

Responsibilities
  • Conduct monthly CCM patient check-ins via phone
  • Assist with patient enrollment into CCM and RPM programs
  • Document patient interactions accurately in the electronic health record (EHR)
  • Review and monitor patient health data received through remote monitoring devices
  • Escalate clinical concerns to the appropriate clinical team or provider
  • Provide patient education regarding chronic disease management
  • Maintain HIPAA compliance and patient confidentiality
  • Coordinate with physician offices and care teams to support patient care plans

Requirements
Qualifications
  • Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) Required
  • Previous experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) is a plus
  • Experience working with electronic health records (EHR systems)
  • Strong phone communication and patient engagement skills
  • Ability to work independently and manage patient follow-ups
  • Bilingual abilities (Cantonese, or Mandarin) strongly preferred
