We're seeking a Licensed Clinical Social Worker (LCSW) to support patients receiving Home Hemodialysis (HHD) and Peritoneal Dialysis (PD) throughout Florida.
This fully remote role focuses on psychosocial assessments, counseling, care coordination, transplant support, and helping patients overcome barriers to successful treatment.

What You'll Do
  • Conduct psychosocial assessments
  • Provide counseling and emotional support
  • Participate in interdisciplinary care planning
  • Support transplant readiness and referrals

What We're Looking For
  • Master's Degree in Social Work (MSW)
  • Active, unrestricted LCSW license in the State of Florida
  • Minimum 2 years of providing psychosocial services, preferably in ESRD, chronic illness, dialysis, or behavioral health experience
  • Ability to comply with Florida Level II AHCA background screening.
  • Proficiency with EMR systems and remote workflow.
  • Strong communication and documentation skills
  • Ability to work independently in a remote environment
Preferred
  • Experience working with dialysis patients, ESRD programs, or home dialysis
  • Bilingual English/Spanish or English/Creole
