Job Description: The LPN Intake Referral Coordinator assumes the day-to-day responsibility for ensuring superior customer service by coordinating patient referrals for home care services. The ideal candidate will have prior home health care experience and/or Intake experience and will be responsible for the smooth flow and continuity of patient care within the department. This position offers a great team-oriented office work environment!

Responsibilities:

  1. MUST present a positive and upbeat phone voice to all incoming calls as this position is the first impression to our referral sources and or patients/family members.
  2. Answers phones for Intake information and enters into the EMR system.
  3. Completes intake form, ICD-10 Coding and routes it appropriately for admission approval. Forwards the intake to PBR for insurance verification/credit approval/Medicare number verification.
  4. Conducts initial nursing insurance authorization on new referrals and as needed, maintaining a tracking log for insurance authorizations and entering into the EMR system.
  5. Maintains referral log for all intakes weekly.
  6. Communicates hard to staff cases to the Intake Manager for further review and consideration.
  7. Assists with in-servicing clinical staff when an educational opportunity presents.
  8. Performs all applicable duties of an LPN in the event that visits are uncovered, and clinical assistance is needed.
  9. Works to develop a positive rapport with all staff members and community resources receiving home care services.
  10. Assists in other branches as needed.
  11. Any duties as assigned by the Intake Manager/Administrator.

Qualifications:

  • Associates Degree preferred
  • Licensed Practical Nurse with current license to practice professional nursing in the State of Virginia
  • At least 2 years of health care experience, preferably in home health care operations.
  • Demonstrates good communication, negotiation, and public skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities

Functional Abilities:

  • Must be able to read 12 point or larger type
  • Must be able to lift, stoop and bend effectively to be able to perform the above listed responsibilities
  • Must be able to hear adequately with no more than an amplifier on the phone and to speak in a manner understood by most persons

Pay: $25.00 per hour-$27.00 per hour
