The Social Services Assistant works closely with the Social Services Director and interdisciplinary team to help residents and families navigate the transition to long-term care, access resources, and maintain the highest quality of life possible. This role provides support with care planning, resident advocacy, discharge planning, and family communication.

Qualifications:
  • Prior experience with social services is required
  • Previous experience in Skilled nursing, long-term care, or similar setting preferred
  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred (or equivalent experience)
  • Strong communication, interpersonal, and organizational skills
  • Compassionate, patient-centered approach
  • Knowledge of long-term care regulations is a plus
  • Ability to maintain confidentiality and professionalism
Key Responsibilities:
  • Assist with admissions, transfers, and discharge planning processes
  • Support residents and families with adjustment to facility placement
  • Help coordinate appointments, transportation, and community resources
  • Maintain accurate social service documentation in compliance with state and federal regulations
  • Participate in care plan meetings and interdisciplinary team collaboration
  • Address resident concerns, grievances, and psychosocial needs
  • Assist with Medicaid applications, insurance paperwork, and resource referrals as needed
  • Provide emotional support and advocacy for residents and families
  • Ensure resident rights are upheld at all times
