) Qualifications
  • Bachelor's degree in Social Work, Psychology, Sociology, Rehabilitation Counseling, Special Education, or a related human services field.
  • Minimum one year of supervised experience in mental health or human services.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to work effectively with residents, families, staff, and community agencies in a fast-paced long-term care environment.

Position Summary
The Social Service Director coordinates and oversees psychiatric rehabilitation services for assigned residents, ensuring compliance with federal, state, and facility regulations. This role supports residents' emotional, social, and behavioral needs through assessments, care planning, counseling, discharge planning, and interdisciplinary collaboration.
Key Responsibilities
  • Complete resident assessments and develop individualized care plans.
  • Facilitate care plan meetings and treatment reviews.
  • Provide counseling, behavioral interventions, and psychiatric rehabilitation services.
  • Maintain accurate documentation, progress notes, and compliance records.
  • Coordinate services with interdisciplinary teams and community resources.
  • Assist with discharge planning, family support, and resident advocacy.
  • Participate in surveys, audits, and quality improvement initiatives.
  • Perform other duties as assigned.
