Summary:
Processes and maintains patient medical records including filing, retrieving and distributing medical documents and reports according to established priorities and procedures in a medical records department.


Responsibilities:
1.Maintains record filing and electronic systems in accordance with established procedures and regulations.
2. Performs concurrent scanning rounding on hospital units daily.
3. Monitors, reconciles, and audits paper based and electronic medical records. Prepares and batches, indexes, analyzes for quality and modifies medical records for the purpose of maintaining a reliable electronic health record.
4. Identifies scanning and indexing errors with timely and accurate correction/modification.
5. Retrieves, delivers and coordinates the delivery of patient medical records for release of information requests, research requests and provider requests and ensures they arrive in a timely manner.
6. Analyzes all bedded, emergency room, outpatient, and ambulatory surgery for chart completion to include provider assistance as necessary.
7. Analyzes and monitors record completion activities of suspended physicians and generates automated incomplete record notices to physicians.
8. Releases approved documents as the legal medical record.
