Overview
Piedmont Rockdale is a 161-bed, acute care, not-for-profit community hospital in Conyers that has provided compassionate, patient-centered care to Rockdale County and surrounding communities since 1954. We offer 24-hour emergency care, plus most major medical, surgical and diagnostic services.
Responsibilities
As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.

  • H.S. Diploma or General Education Degree (GED) Required
  • Associate's Degree or two years of college in human services/health related discipline Preferred

  • 2 years of experience in a healthcare or social services setting Required
