Job Description:
Summary:
The Health Coach is responsible for providing a safe environment for patients to explore their health and lifestyle choices. The Coach will use motivational interviewing skills to review patients' goals and will collaboratively develop healthcare meal, exercise or wellness plans tailored to each patient's needs. The Coach will provide recommendations to reduce or eliminate unhealthy or high-risk behaviors. If the patient has chronic conditions, the Coach will use standardized guidelines to lead and empower the patient in self-management of his care and will consult with the RN Care Manager as needed.
The Health Coach is also responsible for exhibiting effective communication skills while working with patients, families, and other healthcare providers. The Coach must demonstrate the ability to resolve conflicts with patients to overcome their objections about their health and behaviors and be accepting of new ideas, concepts, and approaches to providing education to patients.
Requirements:
Bachelor's degree in a health-related field or Associates' Degree with a minimum of 3 years' experience.

Health Coach certification (or achieve within 12 months of hire)
