Job description

SUMMARYย 

The Case Manager works with the care team to appropriately identify care needs and prepareย clientย for discharge planning. Will work with client employers and insurance payors to coordinate necessary leave of absence documents. The overall goal is to improve outcomes, increase client satisfaction, cost-effectiveness, and promote an informative approach with the client to decrease their concerns around their care.ย 

JOB FUNCTIONSย 

  • Collaborates with the Financial Department to ensure all required insurance payments are collected andย disseminatesย pertinent client information that would prevent payment collections.ย 
  • Participates in assignedย trainingsย and proactively seeks knowledge to build competency.ย 
  • Maintains knowledge of current regulatory and accreditation standards, along with federal, state, and local codes pertinent to this role and addiction treatment.ย ย 
  • Case management duties should include court/legal issues, FMLA/STD, appointments, aftercare, communal services, and speaking with client employers and insurance payers to coordinate necessary documents.ย 
  • Conducts weekly individual case management/counselingย sessionย and provides weekly family check ins.ย ย 
  • Ensures all regulatory standards, requirements, and guidelines, including licensing boards, accreditation authorities, and federal, state, and local regulations are adhered to.ย 
  • Adheres to scheduled work hours with regular and predictable attendance.ย 
  • Collaborates with care team in the development and implementation of clientsโ€™ treatment and intervention programs that they can understand andย comply with.ย 
  • Participates in crisis intervention and conflict resolution actions, including suicide assessments, partner abuse,ย childย and elder abuse screening;ย takes appropriate measuresย to resolve crisis situations; implements mandatory reporting proceduresย in accordance withย regulations (including HIPAA) and policies.ย 
  • Monitors clientsโ€™ conduct throughout the day.ย 
  • Ensures clients are attendingย appropriate sessionsย and are on time.ย 
  • Collaborates with clinical team and attends and participates in clinical team meetings.ย 
  • Conveys clientsโ€™ needs to medical, operational, and other clinical staff.ย ย 
  • Maintainsย a high levelย of flexibility with the daily flow of work tasks.ย 
  • Maintainsย properย chain of command.ย 
  • Assistsย in the facilitation of group sessions as assigned.ย ย 
  • Collaborates with other staff members to plan andย facilitatesย client activities.ย ย 
  • Assistsย in client assessment and clinicalย decisionย regardingย treatment planning.ย 
  • Meets with clients as needed toย monitorย progress and treatment planning.ย 
  • Assistsย inย facilitatingย discharge and aftercare planning.ย 
  • Facilitates family sessions as assigned.ย 
  • Maintainsย appropriate documentationย asย requiredย per Companyโ€™s Policies and Procedures.ย 
  • Maintainย properย chain of command.ย 
  • Attend andย participateย in all staff meetings/in service training.ย ย 
  • Maintainย a high levelย of flexibility with the daily flow of work tasks.ย 
  • Complete all other duties as directed byย leadership.ย 
  • Minimum of Masterโ€™s Degree in Psychology, Mental Health Counseling, Social Work or related field REQUIRED.
  • Two years (2) of working experience preferred.
  • Chemical Dependency experience strongly preferred.
  • Most possess one of the following Licenses: Certified Alcohol and Drug Counselor (CADC), Licensed Clinical Alcohol and Drug Counselor (LCADC), Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW),
