We are seeking a compassionate, organized, and proactive Case Manager to join our dedicated treatment team. This role plays a critical part in supporting our clients through every stage of their recovery journey. The Case Manager will be responsible for completing comprehensive assessments, developing individualized treatment and aftercare plans, assisting with legal and employment documentation, and ensuring all case management tasks align with clinical and ethical standards.

Key Responsibilities:

  • Complete Case Management Needs Assessments and SNAP (Strengths, Needs, Abilities, Preferences) forms for all clients.
  • Create and implement Case Management Treatment Plans tailored to each clients needs.
  • Develop comprehensive Aftercare Plans to support long-term recovery.
  • Facilitate weekly case management groups.
  • Assist clients with FMLA, return-to-work documentation, and legal matters, including short-term disability and Social Security forms.
  • Obtain required signatures for patient documentation.
  • Maintain regular client contact, meeting with clients periodically throughout treatment.
  • Maintain professionalism and model positive mental health behaviors.
  • Protect client rights and confidentiality in accordance with all applicable laws and ethical standards.
  • Respond calmly and maturely in high-stress or rapidly changing situations.
  • Operate independently while adapting to frequent interruptions and shifting priorities.
Qualifications:
  • Minimum of 1 year of experience in a substance abuse treatment setting or related healthcare environment.
  • RADT (Registered Alcohol Drug Technician) required.
  • CADC-I (Certified Alcohol Drug Counselor I) preferred.
  • Demonstrated knowledge and experience in FMLA, disability, and legal documentation processes.
  • Strong understanding of patient confidentiality laws (State & Federal).
  • Familiarity with State regulations, Joint Commission, and Ohio Department of Health standards.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Proficient in verbal and written communication.
  • Strong organizational, time management, and interpersonal skills.
  • Adherence to the Association for Ambulatory Behavioral Healthcare Code of Ethics.
  • Technologically proficient with commonly used clinical documentation platforms and systems.
  • Awareness of workplace safety, drug-free policies, and workplace violence prevention.
