Job Description
As a Home Health RN, you will be responsible for providing direct patient care in a home setting. Your primary duties include assessing patients' health status, developing and implementing nursing care plans, and collaborating with physicians and other healthcare professionals to deliver comprehensive care. You will also educate patients and their families on disease prevention, treatment options, and self-care techniques.
Responsibilities:

Care Planning: Develop individualized care plans in collaboration with the patient's physician, outlining nursing interventions, goals, and expected outcomes.
