PRIDE Health is seeking a local contract nurse RN Hospice for a local contract nursing job in Springfield, Missouri.

Position Overview

Seeking a Case Manager to provide exceptional hospice care to patients facing end-of-life illnesses throughout the Springfield, Missouri region. This role is responsible for managing a patient caseload, coordinating care with the interdisciplinary team, conducting patient assessments, and ensuring patients and their families receive the highest quality hospice services. The ideal candidate is a skilled clinician with strong communication abilities and a passion for delivering patient-centered care.

Schedule & Work Details

  • Coverage Area: Within 50 miles of Springfield, MO
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Hours: 8 hours per day, 5 days per week
  • Pay Rate: $51.00 - $53.00 per hour
  • Productivity Expectation: 25 visits per week
  • Weekday On-Call: 4:30 PM to 8:00 AM, 3 days per week
  • Weekend On-Call: Saturday 12:00 AM through Monday 8:00 AM, 2 weekends per month
  • Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities

  • Serve as the primary RN Case Manager for an assigned hospice patient caseload.
  • Provide skilled nursing care and symptom management for hospice patients in home and residential settings.
  • Perform comprehensive patient assessments and ongoing evaluations of physical, emotional, and psychosocial needs.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
  • Coordinate patient care with social workers, chaplains, hospice aides, physicians, and other healthcare professionals.
  • Participate in Interdisciplinary Group (IDG) meetings every other Wednesday to review patient progress and care plans.
  • Educate patients and family members regarding disease progression, symptom management, medications, and end-of-life care.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Respond to patient and family needs during assigned on-call rotations.
  • Advocate for patient comfort, dignity, and quality of life throughout the hospice journey.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical assessment and patient care skills.
  • Excellent communication, interpersonal, and compassionate bedside manner.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to manage a field-based caseload and travel throughout the assigned territory.

Preferred Qualifications

  • Previous hospice nursing experience.
  • Experience using Homecare Homebase (HCHB) documentation software.
  • Case management experience in hospice, home health, or palliative care settings.
  • Strong organizational and time-management skills.
