Position Overview
As a Community Support Associate at Flourishing Peaceful Families, you will be the bridge between therapy goals and real-world application. This role involves actively engaging with clients in the community, providing support, guidance, and encouragement to help them achieve their therapeutic objectives. We are looking for individuals who are enthusiastic about making a positive impact and empowering clients to navigate and succeed in their day-to-day lives.
Responsibilities:
Community Integration:
  • Collaborate with therapists and case managers to understand clients' therapy goals and objectives.
  • Actively engage with clients in the community to facilitate the practical application of therapy techniques and strategies.
Goal Implementation:
  • Work one-on-one with clients to implement specific therapy goals and interventions in real-world settings.
  • Provide guidance and support to clients as they practice and apply learned skills in everyday situations.
Skill Building:
  • Assist clients in developing and enhancing coping skills, communication skills, and other therapeutic strategies relevant to their goals.
  • Create opportunities for clients to practice and reinforce skills in various community settings.
Collaboration:
  • Collaborate with the therapy team to provide feedback on clients' progress and challenges.
  • Communicate effectively with clients, their families, and relevant community stakeholders to ensure a coordinated support network.
Documentation:
  • Maintain detailed and accurate records of client interactions and progress.
  • Prepare regular reports on clients' achievements and challenges for review by the therapy team.

Qualifications:
  • 18 years of age or older
  • High School Diploma Required.
  • CSA certification
  • 1 year of related experience
  • Strong organizational and communication skills.
  • Familiarity with community resources and services.
  • Ability to work independently and collaboratively within a team.
  • Compassionate and client-centered approach to service delivery.
  • Knowledge of relevant ethical and legal standards in case management.
