Job description

Title: Medical Records Clerk

Status: Full Time, Non-Exempt

Schedule: Day Shift, Monday-Friday


Position Summary:
We are seeking a detail-oriented Health Information Technician to manage and maintain patient medical records, ensuring accuracy, completeness, and confidentiality. This role supports chart analysis, coding, release of information, and departmental operations under the guidance of the Health Information Director.

Key Responsibilities:

  • Assemble, organize, and analyze patient charts

  • Enter diagnosis codes and assist with coding functions

  • Process release of information requests and legal documentation

  • Scan, file, and maintain medical records and departmental forms

  • Assist with audits, studies, and departmental reporting

  • Ensure compliance with HIPAA and maintain confidentiality

Qualifications:

  • High school diploma or equivalent required

  • Minimum 1 year experience in a medical records department; psychiatric experience preferred; relevant education may be accepted in lieu of experience

  • Knowledge of medical terminology and DSM V

  • Strong organizational, computer, and clerical skills

  • Commitment to ethics, confidentiality, and accuracy
