Chart Completion Representative
Sacramento, CA 95833 - Onsite
  • Schedule: Monday-Friday 8am-5pm | 3 months | No patient care
  • Company provides all Equipment

SUMMARY/DUTIES:
  • The Chart Completion Representative I supports Sacramento Home Health by maintaining the electronic health record and ensuring medical records are complete, accurate, filed timely, and retrievable
  • Duties include analyzing charts for deficiencies, notifying providers of incomplete records, retrieving records for physician completion, updating chart control systems, filing and sorting reports, routing documents to appropriate charts, and assisting with record requests
  • This role may merge duplicate health records in EPIC, legacy systems, and paper files, report registration error trends, maintain physician suspension logs and lists, and support reception duties for physicians and department visitors
  • The position requires strong attention to detail, confidentiality, communication, and familiarity with medical record documentation and health information workflows

REQUIREMENTS:
  • HS Diploma or equivalent education/experience
  • Experience with health information management, chart completion, or medical records workflows
  • Experience with electronic health record systems, including EPIC or legacy medical record systems
  • Knowledge of medical terminology, chart analysis, medical record tracking, and abstracting
  • Proficiency in Windows and Microsoft Office applications
  • Ability to manage detailed confidential information and follow patient confidentiality standards
  • Ability to read and comprehend complex instructions, correspondence, and medical record documentation
  • Verbal communication and telephone skills for provider and department coordination
Preferred Requirements:
  • Experience pulling, filing, sorting, and routing high volumes of patient records and documents
  • Experience identifying and reporting duplicate patient health records or registration errors
  • Experience working with physicians on incomplete or delinquent records
  • Knowledge of release of information guidelines
  • Ability to build working relationships with physicians, clinical departments, coworkers, and healthcare professionals
