Job Summary
The Care Management Coordinator, Documentation Improvement actualizes the vision, mission, values, balanced scorecard performance measures for Montefiore Medical Center. She/he maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing's Social Policy Statement. The Care Management Coordinator, Documentation Improvement [CMC-DI] is a Registered Professional Nurse with a sound base of current clinical expertise/knowledge and understanding of coding principles and guidelines. She/he reviews medical records to identify documentation improvement opportunities and then interacts with providers aiming for the provider to document the clarification within the body of the record. The CMC-DI is responsible for improving the overall quality and completeness of clinical documentation so it best reflects a patient's severity of illness and risk of mortality and will therefore support appropriate reimbursement. Extensive interaction with providers at all levels is a large part of this position. Additionally, the CMC-D/I participates in performance improvement (PI) projects designed to continually improve documentation efforts at the medical center.

Qualifications
- New York State License for a Registered Professional Nurse and current registration.
- Bachelor of Science in Nursing or a Bachelor's degree in a related field required- Master's preferred.
- 5 years recent acute care clinical experience preferred, with a minimum of 2 years critical care experience preferred.
- Clinical expertise in disease processes
- Knowledge of clinical medical record coding guidelines/documentation improvement program experience preferred.
- Demonstrated knowledge of federal, state and commercial/third-party regulations related to coding and reimbursement
- Certification in Coding or Documentation Improvement preferred.
- Ability to work independently in a well-organized manner.
- Analytical skills, problem-solving skills, conflict resolution and negotiation skills.
- Demonstrated knowledge of quality/performance improvement tools and techniques.
- Computer skills including data entry, word processing, and electronic messaging. Experience in the use of Windows-based software applications.
- Experience in the use of an electronic clinical information system.
- Excellent verbal, written and interpersonal communication skills, as well as a commitment to interdisciplinary teamwork.
- Excellent time management skills
