Job Description: Member Coordinator and Advocate
We are seeking a compassionate and dedicated staff member to join our team as a member care coordinator and advocate. In this role, you will be responsible for helping the members navigate thru the healthcare maze coordinating and scheduling appointments and benefit support as needed.
Responsibilities:
  • Responsible for outreaching to members to educate on preventative care.
  • Provides educational material and resources to members.
  • Provides education to members on all wellness initiatives.
  • Schedule and coordinate healthcare appointments for patients, including medical, dental, and specialist visits.
  • Helps patients get set-up with remote monitoring devices such as glucometers; blood pressure cuffs; scales, fit bits; and medical alert systems and ensures understanding of how to use devices.
  • Coordinates set-up of all device supplies to be delivered to the patient monthly persunt to the device requirements.
  • Evaluates home safety for those members that require this service; home modification provided if applicable.
  • Coordinates meal service to those members that are eligible for this service and my have been recently discharged from the hospital and would require a short-term meal service.
  • Identifies and sources out community support for those members in need of transportation; etc.
  • Provides support for members when needing help with home care solutions; works with the members preferred vendor to help coordinate service as needed.
  • Supports and helps members navigate their coordination of benefits by working closely with the plan design vendors.
  • Follows-up with members that may require additional support; checks in from time to time to ensure member knows we are available to the member when needed.
  • Develops outreach campaigns that are focused on providing communication to the members of benefits and services.
  • Participates in client meetings where we can share information about the services that are available to its members; provides educational material on all services.
  • Works with the client to ensure needs of members are being addressed that may be overlooked.
  • Expected to outreach to members from a list that management identifies to ensure a laser focus is on members that are in need of additional support.
  • Responsible for revising all pre-certification alerts and ensuring members are provided with provider star rating.
  • Responsible for all follow-up to members after member is discharged from the hospital.
  • Responsible for on-site hospital visits to those members that opt-in to this service.
  • Responsible for coordinating and ensuring that all 3rd party support services are being provided to the members when applicable; home care; etc.
  • Works with company pharmacist to have medication reconciliation review readily available to members if requested.
  • Coordination of social worker should member request service.
  • Expected to always present professionally.
  • Expected to keep all information confidential and follow PHI and HIPAA guidelines at all times.
  • Expected to always show respect to members.
  • Collaborate and work with colleagues; managers and other support staff to review existing processes; revise as needed and develop new processes to maintain the highest standards of customer service.
  • Communicates with HR / client daily to address any concerns or suggestions that may arise.
  • Collaborates with members healthcare providers.
  • Travels to member locations and events as scheduled and needed.
  • Performs other duties as assigned by supervisor.

Requirements:
  • Licensed Practice Nurse (LPN) license; or Certified Nursing Aid (CNA) or Medical Assistant (MA) license; if no longer active that is ok.
  • Minimum of [3] years of experience preferably in a healthcare coordination or support role.
  • Excellent communication and interpersonal skills, with the ability to listen and provide emotional support.
  • Strong organizational skills and attention to detail, with the ability to effectively schedule appointments and maintain accurate records.
  • Humble and personable demeanor, with a genuine desire to assist and support.
  • Ability to work independently and collaborate effectively with healthcare providers.
  • Ability to work with Office products; Word, Excel, PPT
