Supervised Visitation & Safe Exchange Care Coordinator

North Homes Children and Family Services (NHCFS) is seeking a dedicated and compassionate Care Coordinator to join our Supervised Visitation and Safe Exchange (SVSE) team in Bemidji, MN. The SVSE Care Coordinator is responsible for facilitating program referrals, coordinating with the referring party and parent(s), completing intake/orientation with the parent(s), and scheduling visits.

Job Duties Include:

• Point of contact for program referrals
• Obtain required information and documentation for referral
• Interview and assess family history and strengths for new referrals
• Coordinate with referring party, parent(s), and other collateral contacts
• Complete intake/orientation with new referrals
• Assign cases and schedule visits with Visitation Monitor
• Adequately plan and prepare for each visit
• Document visits, activities, and other contact with family or collateral contacts within expected timeframe
• Review Visitation Monitor documentation and files
• Review case updates with program supervisor
• Maintain building security and cleanliness
• Follow all program and agency policies and procedures
• Complete assigned trainings that are required for the position and agency
• Attend and participate in consultation, program, and agency staff meetings
• Batterer’s Intervention Program (BIP) Facilitator
• Provide psychoeducation group weekly
• Provide monthly BIP orientation for new clients
• Attend BIP Facilitator meetings monthly
• Duties of Visitation Monitor
• Other duties as assigned

To be employed as a SVSE Care Coordinator, an individual must meet all the qualifications:

Minimum Qualifications:

• At least 21 years old and have a valid driver’s license
• High School diploma or GED and at least 3 years of experience working with human services, specifically with victims of domestic violence/intimate partner violence/sexual assault; or Bachelor’s degree in human services or related field
• Ability to pass a DHS background study and meet applicable requirements and regulations
• Flexibility to work evenings, weekends, and some holidays

Necessary Skills, Knowledge, and Abilities:

• Strong communication, problem-solving, interviewing and assessment, conflict resolution, and report writing skills
• Working knowledge of child development, behavioral health, trauma-informed care, family systems relationships, behavioral management techniques, community resources, recreational activities
• Ability to remain non-judgmental in regards to lifestyle, culture, race and/or economic status
• Ability to establish and maintain effective working relationships
• Ability to maintain confidentiality
• Basic computer skills, including Microsoft Word and Outlook Email
• Ability to sit or stand for long periods at a time and work in a variety of settings, including indoors and outdoors
