Job Description
I. JOB SUMMARY: To evaluate resident’s rehabilitation & therapy needs, develop an appropriate plan of care and provide occupational therapy treatment in any or all areas the department serves. The plan of care will be documented for communication with the resident’s M.D. and all members of the multidisciplinary care team in the form of an initial evaluation, treatment, progress, re-evaluation and discharge notes. Occupational Therapist will work hours (including weekends and holidays) and sites as necessitated by the department’s needs.

II. PRIMARY JOB DUTIES:
- Evaluate resident and develop a plan of care, which is functional, and goal orientated in addressing resident needs.
- Follow through on re-evaluation of resident to determine response to treatment and adjust the plan of care as needed.
- Prepares and maintains all documentation as required by the outside agency or area of patient service.
- Discuss the resident's progress with the attending physician, family, nursing, and other allied health professionals as needed.
- Assists in discharge planning and instructs resident and family in a home program and documents teaching.
- Performs a variety of supervisory and educational activities, such as supervising activities and scheduling of the occupational therapist assistants. Also, supervises and instructs occupational therapy students from affiliated schools.
- Presents in-service education programs to department and nursing home staff.
- Attends designated meetings, in-service programming and seminars.
- Maintains Occupational Therapy license as required by the NYS Department of Education.
- Recommends appropriate adaptive equipment, assistive devices and rehabilitation equipment to the resident and physician.
- Assists and orientates new employees in departmental/facility forms and procedures.
- Maintains work environment in a manor conducive to safety.
- Performs other related duties as required.
- Selects appropriate environment/equipment.
- Positions & drapes resident for maximum comfort, safety and effectiveness.
- Reacts to change in resident's status in a given treatment session.
- Administers chosen treatment according to established guidelines.
- Prepares and submits relevant quality assurance reports.
- Other duties as assigned by Supervisor

- Demonstrates safe resident care.

- Uses good body mechanics for self and residents.
- Demonstrates knowledge of indications and contraindications in

Education:
- M.S. Degree in Occupational Therapy
License/Certification:
- Current N.Y.S. Occupational Therapy License registered by AOTA
Experience:
- Prefer one (1) year of relevant long term care experience.
