OVERVIEW

Plan, organize and administer the daily operations of the Adult Day Health Care Program. Supervise staff, conduct educational public speeches, assess client needs, and budget program expenditures. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

ESSENTIAL FUNCTIONS

  • Maintain and establish policies and procedures for program operations and clinical services that comply with contractual and certification standards
  • Responsible for recruiting, hiring, training, evaluating, providing constructive performance feedback, and separating employees
  • Track and assess all individuals referred for program services eligibility
  • Reconcile all referral information provided to determine functional status and client activity goals
  • Provide accurate documentation of services and prepare statistical reports for funding sources in accordance with contracts
  • Promote referral relationships with outside agencies and participate in the development of needed resources through community education and program communications
  • Provide ongoing assessment of client and program needs, and the development of services to meet those needs
  • Teach and train clients and caregivers in the provision of care
  • Administer or assist clients in taking medication as ordered by physicians
  • Track the care and treatment goals of participants through accurate and up-to-date charting
  • Participate in case conferences and orientation
  • Ensure compliance with Child and Adult Care Food Program (CACFP) requirements, i.e., directing and managing program, record keeping, report completion, training, etc.
  • Coordinate team members in the planning of participant care
  • Supervise Program Aides and volunteers
  • May drive agency or personal vehicle on company business

MINIMUM REQUIREMENTS

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Executive Director of Human Resources.

  • Current Arizona Registered Nurse license
  • 2 years of directly-related experience
  • 2 years of supervisory experience
  • Ability to obtain health care certificates including CPR and Food Handlers/ServSafe Certificate
  • Proven computer skills utilizing Microsoft Office Suite
  • Proven strong communication skills - oral and written are clear, concise, and in an organized fashion using appropriate style, grammar, and tone
  • Proven ability to facilitate an interdisciplinary approach for providing quality service to participants
  • Ability to stoop, climb, bend, and reach

REGULATORY

  • Must be at least 18 years of age
  • Valid driver license, proof of insurance, and 39-month motor vehicle report
  • Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid)
  • Pass pre-employment drug screen (incudes marijuana, regardless of recreational use laws) (employer paid)
  • TB screening (employer paid)

DESIRED QUALIFICATIONS

  • Knowledge of Older Americans Act: Titles IIB, IIIC, IIID and IIIF, TXX, ALTCS (Title XIX)
  • 1 year of experience in responding to emergency situations, including the ability to assist with possible evacuation of participants
  • Additional years of supervisory experience
  • Additional years of directly-related experience
