JOB SUMMARY

This position is responsible for supporting IHB Diabetes Program, as well as other Health programs through professional communication with patients and staff, organized event planning, and efficient management and analysis of clinical data.

ESSENTIAL FUNCTIONS
  • Participates as an integral member of the Health Care Home team; collaborating with various IHB staff to provide seamless patient care.
  • Collaborates with IHB's health programs to support patient care through various communication strategies; scheduling meetings, scheduling patients' appointments as it relates to the access of care.
  • Taking meeting minutes and distributing them to applicable staff in a timely manner.
  • Completes inventory of purchased items on a regular basis for health program supplies and incentives, including clinical diabetes supplies and grant incentives.
  • Support diabetes clinic to include purchase and keep current inventory for various health program.
  • Maintain accurate and current quality measures for team.
  • Collect and analyze data from events, leading efforts in program audits, grant reporting and grant activities.
  • Lead class and event preparation, marketing décor and assisting in planning meal preparation.
  • Work with IHB's Dietitian to order food and supplies for classes and events, following appropriate purchasing and reconciliation procedures and budget as well as IHB's Healthy Foods guidelines.
  • Work with Health Programs staff to create/update flyers to promote health activities, events, and health education materials.
  • Collaborate with Care Coordinators and partner with health care agencies and community resources to implement, coordinate and manage program events.
  • Provide excellent customer service to patients, peers, leadership, and community partners.
  • Complete and submit all reports in a timely manner, ensuring accuracy and thoroughness (e.g., audits, grant reports, internal reports, etc.).
  • Assist with 340B calls to pharmacy to ensure written correctly.
  • Setting up meetings and reserve rooms for all diabetic programs, and other health program activities and/or events.
  • Responsible for supporting team and patients in day-to-day activities.
  • Perform other job-related duties assigned by supervisor.


ADDITIONAL RESPONSIBILITIES
  • Assist in workflow improvement related to job activities.
  • Attend staff and department meetings.
  • Take meeting minutes and distribute to staff.
  • Create documents to improve clinic and patient workflow.


PLANNING ACTIVITIES
  • Understand current requirements of grant and funding entity.
  • Participate in Medical Department planning efforts to meet departmental and organizational goals.
  • Ability to establish and maintain productive relationships with staff from all IHB departments and community agencies.


EVALUATION ACTIVITIES
  • Participate in annual proficiency testing to evaluate necessary skills.
  • Participate in performance improvement evaluations and activities.


KNOWLEDGE, SKILLS, AND ABILITIES
  • Ability to implement projects from start to finish thoroughly completing all phases, meeting deadlines.
  • Ability to work efficiently in a fast-paced environment.
  • Knowledge of/or ability to be self-motivated to learn basic health screening guidelines.
  • Knowledge and application of office and clinic procedures.
  • Excellent people, oral/verbal, and written communication skills.
  • Easily motivated, creative, engaging, and enthusiastic.
  • Ability to type more than 50 words per minute with accuracy.


EDUCATION AND EXPERIENCE
  • Associates-degree in a health-related field, preferred.
  • Ability to be self-motivated to work efficiently with multiple people on multiple projects, meeting deadlines.
  • Excel at working in a clinic environment, working professionally with medical providers, clinic staff, community partners and patients; 2+ years' experience preferred.
  • Knowledge and respect of American Indian culture and other culturally diverse populations served by IHB.
  • Reliable transportation, current license in good standing with a good driving record required.

This position is access restricted to PHI needed to provide health education and carry out health care operations in assigned clinical area.

PHYSICAL DEMANDS

The work requires some physical exertion such as long periods of standing, walking, and recurring bending, crouching, stooping, stretching, reaching, or similar activities. The work may require specific, but common, physical characteristics and abilities such as above average agility and dexterity. An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, blood borne pathogens, physical hazards (e.g. needle pricks, etc.) and chemical hazards
