Call-Out Response Team



Hours: Monday - Friday

Status: Full Time

Reports to: Homeless Response Team Director


Purpose:

Under the direct supervision of the HRT Director, this position will provide outreach and engagement services to people experiencing homelessness in Clark County. The Call-Out Response Team will respond to requests for service and provide in-the-field crisis intervention, assessments, referral, and connection to services for those who have been living in areas not meant for human habitation.



Duties, Functions, and Responsibilities:

  1. Will provide hands-on assistance to individuals living on the street or in a shelter.
  2. Provide case management and crisis intervention services, medication monitoring, social support, assistance with everyday living needs, and access to medical care and employment assistance.
  3. Will advocate on behalf of, and assist clients in accessing community resources and support, to ensure equity and appropriate services are offered.
  4. Conduct assessments to determine a client's current and potential strengths, weaknesses and needs.
  5. Transport clients to necessary services and treatments provided in the community.
  6. Provide crisis intervention, to promote stability, through direct intervention and mobilization of needed supports and services.
  7. Provide responder assessments to determine the number of homeless households in a given geographical area.
  8. Assess and determine a person's eligibility, current and potential strengths, weaknesses and needs towards placement in a stabilizing treatment or housing situation.
  9. Monitor outcomes of referrals, to ensure that each client achieves and/or maintains stability.
  10. Routinely monitor and frequent geographical areas that continue to have households who reside in places not meant for human habitation.
  11. Maintain accurate documentation of services provided to clients, in real time (daily), into HMIS.
  12. Participate in multidisciplinary teams to report and review client progress.
  13. Actively participate in Continuum of Care and community planning to end homelessness.
  14. Report outcomes to the County and other designated organizations, as required.
  15. Other duties, as may be assigned.

Knowledge, Skills and Abilities:

  1. Knowledge of local social service agencies and programs, as they pertain to the homeless population.
  2. Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations.
  3. Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
  4. Ability to maintain order and exercise appropriate judgment in crisis situations.
  5. Is flexible and can quickly adjust to shifting priorities.
  6. Establish and maintain effective working relationships with clients, fellow employees, other agencies, and the general public.
  7. Knowledge of social service work, interviewing/referral techniques, and applicable laws, statutes and regulations.
  8. Must have the ability to follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
  9. Experience with and understanding of homeless client's issues.

Qualifications, Education and/or Experience:

  1. High School diploma or GED
  2. Mental health related bachelor's degree, or 2-years of experience providing mental health services.
  3. Must pass a background check and pre-employment drug screening. Random drug testing is performed.
  4. Must have a valid NV driver's license and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
