The mission is being accomplished through a variety of programs and activities, such as: medical outreach and referrals for medical care, HIV services and drug treatment; health education; psychosocial assessment and intervention; primary medical care; family planning services; and research.

Position Purpose:
The Patient Retention Specialist will support the professional care-team staff, consisting of a Registered Nurse (RN) and a Master’s level Social Worker (MSW), in implementing the Medical Care Coordination (MCC) program and delivering medical and non-medical case management activities to patients. Responsibilities include identifying eligible patients, connecting them with medical services, and ensuring their retention in medical care. The specialist will also provide referrals to support services and follow-up with clients assigned to the MCC team, ensuring successful linkage to services and effective communication of results to the team.

Principal Responsibilities:

  1. Establishes partnerships with HIV testing sites to facilitate the connection of newly diagnosed patients with the MCC team.
  2. Identifies MCC-eligible patients based on registration records.
  3. Registers and conducts intake assessments for eligible patients.
  4. Performs data entry tasks in DHSP’s data management system and population health databases.
  5. Participates as a member of the healthcare team, interviewing patients to gather information about their home environment, family dynamics, and personality traits relevant to their illness, treatment, and recovery.
  6. Engages in team conferences, sharing social assessments and evaluations with other healthcare disciplines to develop comprehensive care plans.
  7. Conducts ongoing interviews with patients to identify any new needs not previously addressed by the MCC team, informing team members to update care plans accordingly.
  8. Assists patients and the MCC team in addressing socioeconomic barriers that affect patients’ access to and adherence to HIV care and treatment, coordinating referrals for services such as housing, transportation, financial support, and benefits.
  9. Utilizes electronic medical record queries to monitor patient-level health outcomes and identify patients in need of MCC.
  10. Assists in documenting patient referrals and monitoring activities in patient files.
  11. Maintains accurate patient files to support continuous quality management reviews by DHSP.
  12. Performs other duties as assigned.

Requirements:

  1. Bachelor’s degree in Social Work preferred or related Human Services degree or equivalent experience with approval of DHSP.
  2. Experience in case management with HIV or similar population can be substituted.
  3. Experience with providing HIV/AIDS services is preferred.
  4. Two years of experience in a health care setting.
  5. Evidence of recent education in HIV/AIDS related health services.

Preferred:

  1. Bilingual (English/Spanish) proficiency.
  2. Six (6) months of experience working in a clinical setting.
  3. Strong interpersonal skills and the ability to collaborate effectively with other members of the clinical and case management team.
  4. Capable of researching, identifying, and accessing community referrals.
  5. Understanding of patient confidentiality and awareness of the legal consequences of non-compliance.
  6. Excellent verbal and written communication skills.
  7. Proficient in computer usage.
  8. Knowledgeable about substance use, homelessness, mental illness, and other issues affecting the client population.
  9. Organized, with the ability to maintain client files, charts, and referral records.
  10. Basic computer proficiency, including word processing, data entry, and internet research.
  11. Strong quality management skills to ensure compliance with all guidelines and regulations.
