Starting rate $28, full time, Monday- Friday 8am- 4pm with flexibility.

• Actively advocate with physician and hospital staff to ensure the individual receives the highest level of care.
• Document and review individual records to assure appropriate medical follow-up is provided.
• Active case management role, QA checks
• Residential home visits per regulation, medical oversight and support.
• Regular site visits to day programs and oversight to areas in accordance with OPWDD regulations.
• Staff training.
• Medication review.
• Complete regular documentation.
• Oversee diets and nutrition.

• Valid NYS RN license
• Drivers license
• Experience with persons with disabilities preferred
