Job Description

SUMMARY:

The Adult Day Center Manager is responsible for day-to-day management of a day program for seniors with Alzheimer’s disease, related dementia or physical frailty and for the provision of individualized services in a center-based day habilitation setting. The Adult Day Center Manager coordinates with Care Coordinators identifying individual needs for day habilitation services, and assures that services meet federal, state, and agency guidelines and regulations. The Adult Day Center Manager is responsible for coordination of care, developing a Plan of Care and coordinating services and supports for seniors and adults receiving Adult Day services. The Adult Day Center Manager is responsible for recruiting, orientation, training and supervising staff within the adult day and day habilitation setting. The Adult Day Center Manager will assist with coordination and provision of services. Staff supervision and program management skills are essential to this position. The Adult Day Center Manager works with the Senior Services Director on program development and staff training.

DUTIES AND RESPONSIBILITIES:

  1. Promotes and ensures a safe nurturing environment for individuals attending the Adult Center, assuring that safety precautions are observed.
  2. Manages and supervises Adult Day Center staff. Assures adequate staffing for program needs and a team atmosphere for staff.
  3. Conducts Adult Day Center intake, coordinates Plan of Care and identified services.
  4. Develop, implement and maintain Plan of Care for Adult Day Center participants as well as perform reassessments annually or as necessary.
  5. Assists in developing the Plan of Care for Day Habilitation participants addressing goals and objectives for center-based activities.
  6. Assists in evaluating program services and designing and implementing program development objectives.
  7. Responsible for on-going case management with participants.
  8. Utilize crises intervention and problem-solving skills.
  9. Assures program activities meet individual and collective needs of participants and families, including coordinating with community agencies for special events.
  10. Conducts interviewing and hiring, orientation specific to adult day services and day habilitation services and performance evaluations for staff.
  11. Monitors the amount, duration and scope of services provided within each Plan of Care.
  12. Manages the overflow of maintenance of Adult Day Center facility, monitors general cleanliness and orderliness and availability of proper supplies and equipment.
  13. Prepares reports on special events.
  14. Assists in preparation of Adult Day Center portion of the department’s bimonthly newsletter.
  15. Maintain accurate information in department’s database.
  16. Requests or performs emergency care as needed.
  17. Inputs participant information and case notes into electronic medical record, in accordance with agency procedures, state and federal requirements.
  18. Implements all agency policies as appropriate.
  19. Attends training seminars, in-service sessions, and staff meetings.
  20. Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations.
  21. Monitors staff training compliance, case note documentation and other staff requirements using an electronic medical record (EMR).
  22. Provides other duties as necessary.

MINIMUM EDUCATION AND EXPERIENCE:

  1. Bachelor’s degree in nursing, Gerontology, Human Services, Social Work or related field preferred with background in long-term care and delivery of community-based services. Experience and/or specialized education may substitute for degree requirements on a year for year basis.
  2. Two years of supervisory experience, preferred.
  3. Four years full-time or equivalent part-time experience working with seniors who experience Alzheimer’s disease and related dementia.
  4. Four years full-time or equivalent part-time experience working with individuals experiencing developmental disabilities and/or physical disabilities.
  5. Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Demonstrated skill in staff supervision and program management. Demonstrated ability to communicate effectively with participants and families, assess needs and coordinate individual services. Ability to evaluate quality of care and services being provided. Excellent writing and verbal skills. Ability to communicate effectively with staff and assess training needs. Must have reliable transportation and maintain a good driving record. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Ability to learn new software and hardware applications, including Microsoft Office applications and accounting software.

LICENSES/CERTIFICATIONS REQUIRED:

Alaska Driver’s License

Certification in Adult CPR

Certification in First Aid

Safety Care Certification

IMMUNIZATION REQUIRED:

Annual PPD Test

Hepatitis B immunization (or) Waiver

WORKING CONDITIONS:

Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

PHYSICAL ACTIVITIES:

Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.

Talking: Expressing or exchanging ideas by means of the spoken word. Those activities which demand detailed or important instructions spoken to other workers accurately, loudly or quickly.

Driving: Minimum standards required by State Law (including license).

ENVIRONMENTAL CONDITIONS:

None: The worker is not substantially exposed to adverse environmental conditions (such as typical office or administrative work)
