The Community Health Worker, in collaboration with the transdisciplinary team, provides and/or facilitates the acquisition of economic, social, and emotional support services to patients and their families. This position is responsible for psychosocial consultation to other agency personnel and transdisciplinary team members, referrals to outside agencies, and provides direct services to patients and their families. The Community Health Worker maintains complete and accurate patient records following established Western Reserve Care Solutions standards of documentation practice and regulatory and licensure requirements. Must be CHW certified.

WHAT YOU WILL DO:

  • Follows up and interacts with patients/families/caregivers and employees in a courteous and professional manner always.
  • Participates in the transdisciplinary team and provides direct psychosocial, emotional, cultural, and spiritual support to patients and families.
  • Uses professional background and skills to assist the team.
  • Coordinates team efforts with appropriate hospital, hospice, palliative care and/or facility departments to ensure care coordination, smooth discharge planning and/or transition to appropriate services.
  • Assists patients/families/caregivers and staff in acquiring and utilizing internal and external resources.
  • Provides education, support and follow-up as needed with patients.
  • In collaboration with the IDT, trains and reinforces training to the caregiver(s), based on identification of training needs
  • Delivers health information using culturally appropriate terms and concepts.
  • Respects diverse opinions and experiences in interactions.
  • Collaborates directly with the transdisciplinary team to identify a patient plan of care and needed follow up, support.
  • Participates in meeting the patient plan of care.
  • Documents patient care following established Western Reserve Care Solutions standards of documentation.
  • Adheres to established policies and procedures and all regulatory and legal requirements and practices within licensure.
  • Consistently follows all departmental and organizational protocols and practices to ensure service excellence is achieved with both internal and external customers, and that boundaries within the scope of practice are not exceeded.
  • Works closely with medical providers and social service agencies to provide care coordination and connection to resources.
  • Maintain up to date knowledge of community resources in a changing healthcare environment.
  • Adjusts to changes in workload and schedules based on changing departmental and organizational priorities.
  • Ability to participate in patient care per the direction of registered nurse based on the Ohio Revised Code

SUCCESS CAPABILITIES:

  • Graduation through an approved training program.
  • Maintain certification through the Ohio Board of Nursing
  • Knowledge of hospice and palliative care preferred.
  • Ability to practice in accordance with Western Reserve Care Solutions clinical standards of practice, policies, and procedures.
  • Knowledge of community and social services resources across the program footprint.
  • Ability to establish and maintain effective working relationships with employees and the community at all levels.
  • Ability to demonstrate excellence in written and oral communication.
  • Ability to demonstrate positive customer service responsiveness.
  • Must be flexible and have problem solving skills, prioritize and demonstrate healthy coping strategies.

CONDITIONS OF EMPLOYMENT:

  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.
