Job Title: Case Manager

Program: Outpatient Services

Location: 700 Airport Road, Lakewood New Jersey

Position Type: Full Time

Job Description/Summary

The Case Manager provides community-based outreach to address barriers to care and services for behavioral and physical health, as well as needs related to social determinants of health. This person will provide referrals, linkages, advocacy, and collaborate directly with multidisciplinary treatment team. The Case Manager shall possess cultural humility and how to address common barriers and risks associated with underserved populations.

Responsibilities

  • Linking to appropriate care and resources in the community including resources that address specialized needs, such as agencies providing services related to chronic and acute health problems and problems stemming from citizenship status.
  • Provide timely documentation into client’s clinical records noting any therapeutic participation, responses to interventions, client advocacy and service coordination.
  • Provide case management services to ensure that individuals are both informed about and able to access the full range of medical and behavioral health treatment or other support services related to social determinants of health including legal, financial, educational, vocational, or employment counseling.
  • Possess a working knowledge of motivational enhancement strategies and is able to use these and other engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthy lifestyle.
  • Demonstrate ability to assess client needs, and provide interventions that are timely and appropriate.
  • Outreach “high risk” clients within 24 hours and “at risk” consumers within 48 hours of case assignment.
  • Contribute to the sustainability of the PBHG by considering resource gaps and problems of service delivery, reporting any need or development opportunities to leadership.
  • Engage new persons served into the program and works to re-engage persons who have decreased their participation.
  • Actively seek supervision to discuss pertinent clinical issues and intervention strategies to foster consistency and enhance progress toward wellness for all individuals served.
  • Act as an advocate and liaison and provides community linkages, advocacy and referral sources for persons served.
  • Demonstrate understanding about the impact of mental illness and/ or substance abuse on individual health and wellness, and involve support networks and families in the plan of care whenever appropriate.
  • Seek appropriate consultation with clinicians and supervisors when persons served exhibit danger to self or others.
  • When appropriate, coordinate or assist with crisis intervention and stabilization services.
  • Maintains required CPR and first aid education/certification.
  • Completes annual E-Learning modules as required.

Qualifications

  • Education: Bachelor’s Degree in the human services field, such as Social Work, Psychology, Counseling, or a related discipline

Competencies:

  • Possesses strong organization and time management skills.
  • Demonstrate a Trauma Informed approach to all client interaction.
  • Communicates and works effectively within a team
  • Work effectively with the full spectrum of mental health disorders.
  • Ability to communicate in English, bilingual in Spanish is a plus.
