Job Description

JOB SUMMARY: This position requires the supervision of the plan of care for personal care clients, as well as the aide assigned to the client home. The position requires considerable travel throughout the area and a visit to each client according to agency policy. Some travel out of the area is required, also. Detailed visit and supervisory records must be maintained. May be required to lift up to 50 lbs. on occasion.

JOB RESPONSIBILITIES:

  • Maintains supervisory and nursing support to the Personal Care Program to maintain the program in accordance with established policies and procedures.
  • Develops monitors and maintains plan of care for each client assigned.
  • Recruits, employs and trains personal care aides in a ,manner consistent with Agency policies and procedures, and guidelines of the Virginia Department of Medical Assistance Services (DMAS).
  • Maintains visit schedules for aides to ensure maximum efficiency and effectiveness.
  • Maintains visit schedule to client homes, using personal automobile if necessary, to ensure that each client is visited, per agency policy.
  • Participates in and provides on-going training for staff and personal care aides in conjunction with the Director of In Home and Family Support Services.
  • Ensures program compliance standards and ensures that all procedures and policies are in compliance with DMAS standards.
  • Develops and maintains close working relationships with local Departments of Health, Departments of Social Services, the Pre admission Screening Teams and public and private providers of in home and home health services.
  • Participates in the education of the public to promote the availability of the program, with the goal of reaching all the eligible clients in need of personal care.
  • Represents department at PACE morning meeting, in absence of PACE Home Care Coordinator.
  • Assists PACE Home Care Coordinator with six month and annual reassessments for personal care recipients enrolled in PACE.
  • Performs those administrative duties consistent with overall goals and mission of the Agency to promote maximum efficiency and effectiveness.
  • Reviews all paperwork for accuracy and integrity.
  • Assists with Agency fundraising events, the Emergency Services program and assists with the publication, as requested, for the Mountain Empire News.
  • Participates in Agency staff meetings, when scheduled.
  • Responsible for primary coverage of phones, within department, one day per week. Also assists with answering phones, as needed.
  • May occasionally be required to lift, carry and arrange up to 50 lbs., over short distances.
  • Other duties as assigned by the Director of In Home and Family Support Services or the Executive Director.
  • Maintains up to date knowledge of given area of responsibility.
  • Regularly reviews journals, attends meetings, etc., as provided by the Agency to ensure current knowledge of areas of responsibility.
  • Researches and/or recommends new processes, procedures and applications, which can better serve Agency or client needs.
  • Displays an interest and enthusiasm in on going Agency sponsored development activities; supports Agency efforts with consistent participation and attendance.
  • Develops and maintains Employee Development Plan to ensure continued professional growth and development.
  • Maintains professional presence when representing Agency. Exhibits degree of professionalism consistent with standards of performance, behavior and appearance.
  • Performs on the job to the best of ability at all times.
  • Exhibits professional presence, consistent with Agency’s standards and values at all times.
  • Interacts with Agency personnel to facilitate positive, productive working relationships.
  • Provides positive assistance and support at all times when dealing with staff and clients.
  • Maintains highest degree of confidentiality in all aspects of client relations and Agency operation.
  • May act as a substitute member on the IDT team for PACE program.

SUPERVISORY RESPONSIBILITIES: Personal Care Aides

QUALIFICATIONS:

  • Current RN license to practice in Commonwealth of Virginia
  • Two years related clinical experience as an RN.
  • Valid Driver’s License
  • Satisfactory work history, as evidenced by two satisfactory reference checks from previous employers.
