Senior Healthcare Administrator- PACE Site
RN License Required
under the direction of the Care Delivery Regional Director or designee, supervises all day-to-day operations of the PACE site, including but not limited to Clinic, Day Health Center, Front Desk, Facilities, Interdisciplinary teams, and coordinates the multi-disciplinary integrated care in the community.
JOB RESPONSIBILITIES:
Balances & allocates IDT and related resources throughout the site and ensures adequate team assignments and coverage.
Monitor and ensures timely, effective development and execution of personalized care plans that result in improved satisfaction & outcomes for all participants.
Provides oversight of the provision of services that impact participants’ care.
Coordinate in-service trainings and management directives to support the development and advancement of team members.
Maintain knowledge of all applicable CMS, DOH, and PACE-specific guidelines, including organizational and departmental policies and procedures.
