DSP - Direct Service Provider

JOB SUMMARY

Provides delivery of individualized care in our medical model adult day care program, in a respectful, thoughtful and dignified manner. Fulltime Monday - Friday, 8 am - 4:30 pm no weekends or holidays.

PRIMARY DUTIES Provides care for individuals according to their plan of care.

1. Promotes the maximum level of functioning for all.
2. Reports unacceptable behavior or actions to program nurse.
3. Identifies specific needs and asks for direction to meet these needs.
4. Works closely with other DSP’s and the program nurse.
5. Accepts assignments as directed by the nurse in a positive manner.
6. Provides all duties as deemed necessary by the nursing staff.
7. Promotes participation in activities and evaluates their interests in the program activities.
8. Demonstrates a positive role model for all.
9. Monitors hydration and food intake.
10. Monitors toileting, and if appropriate places on a toileting regime.
11. Promotes safety and security at all times.
12. Reports falls and injuries immediately to the nurse.
13. Complies with the current manner of occurrence reporting.
14. Knowledgeable of all safety requirements of fire, tornado, earthquake, and medical emergency policies.

OTHER ESSENTIAL DUTIES

1. Reports all physical and mental changes to the nurse, in a prompt manner.
2. Monitors for signs and symptoms of infection, fever, malaise, poor intake; dysuria, cough, or cold symptoms.
3. Continues professional development by attending in-service, seminars, or other means of formal and informal education
4. An active team member of the program with ongoing input regarding staff issues, client issues, and furthers development of the adult day health program.
