As an LPN Intake and Scheduling Coordinator, your responsibilities will include;

Acts as a liaison between field staff, hospital discharge planners, insurance company reps and office staff (20%).
Providing administrative support to Clinical Management staff (10%).
Being responsible for intake, staffing and following up of routine referrals, client’s file maintenance and management (70%).
Performing PPD'S and hosts nursing orientations and performs skills checks for HHA.
Verifying insurance information on referrals and maintaining logbook.
Screening files for deficiency and any other issues and following up with corrections.
Creating appropriate logs to monitor performance improvement.
Maintaining schedules and geographic coverage for staff.
Making HHC-3000 data entries.
Ensuring information is communicated to field staff, office staff, and customers in a timely manner.
Participate in on call duties.
Performing specific tasks as assigned.

What you will need:

Current Active PA LPN License required.
Experience in home health or an agency environment highly desirable.
Exposure to best practices in the fields of pediatric, geriatric, and home health services.

Job Type: Full-time
Salary: $55,000.00 - $65,000.00 per year
