POSITION SUMMARY:

This is a professional position providing intensive case management in the Together We Rise program under the Families First Division. The Families First Division was created as a result of the Family First Prevention Services Act (FFPSA) which is focused on keeping children safely with their families to avoid the trauma that results when children are placed in out-of-home care. Families First Division operates with the understanding that children do best in a strong family, and that children and their families do best with support from their communities. To increase the number of children who can remain safely at home with their families, the law provided families with greater access to mental health services, substance use treatment, and/or improved parenting skills. The Families First Division ensures families access to mental health, substance use treatment and supportive services focused on strengthening families and proactively supports those children at risk of abuse, neglect and separation from their family. This position works in a team approach to decrease the number of children entering the Child Welfare System by utilizing a solution-focused, family centered approach and evidenced based practices to assist families in meeting needs and goals while preventing further incidents of abuse, neglect and abandonment. The intensive case manager will work with the other members of the team to provide a supportive, empowering and respectful relationship with families to facilitate change and emphasize the need to take action by providing both concrete services and family interventions. Requirements of this position include assessing, linking, coordinating and monitoring services from mental health, social and educational aspects, and providing those services within the home or in the community. The Intensive Case Manager is focused on the "here and now" and works in a strengths based, honest, and direct approach, collaborating with the parents to address barriers to include housing stability, connection to health plans, time management, and parenting skills. The intensive case manager will be an active participant at the Integrated Decision Team process and will provide face to face visits with the family according the frequency required in order to mitigate risk. This position is required to complete dual entries into two separate systems: the agency electronic health record and the Florida Safe Families Network (FSFN). DFL prides itself on the fact that it is a trauma informed care services provider. This position requires Behavioral Health Certification through the Florida Certification Board (FCB). Hours are flexible, evening and weekend work is required. This position will work closely with members of the Together We RISE team such as: Counselor, Peer, and Supervisor and Clinical Supervisor and in collaboration with other community service providers externally to provide complete comprehensive services.

Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.

EDUCATION EXPERIENCE/REQUIREMENTS:

  1. Must hold at least a Bachelor's Degree Social Work or related area of study from an accredited university; or a Master's Degree in Social Work or related area of study a minimum of 2 years of experience working with at-risk populations to include families experiencing substance misuse, domestic violence, mental health, and emotionally disturbed children.
  2. Previous child welfare experience and certification is strongly preferred.
  3. Must acquire knowledge of community resources
  4. Knowledge of child development, including physical, mental, emotional and cultural issues preferred.
  5. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly.
