For fifty years, Bayview Senior Services, (aka Bayview Hunters Point Multipurpose Senior Services) has responded to community needs and served as a catalyst for neighborhood activism and advocacy. Programs are designed to enhance and retain the health, quality of life, and culture of African American elders who remain as long-standing members of communities in San Francisco.


We are searching for a highly motivated Physical Therapist to join our team!


The role of the Physical Therapist at the Bayview Hunters Point Adult Day Health Center is integral to facilitating the wellbeing and autonomy of seniors and individuals with disabilities. Licensed by the California Department of Public Health and certified as a MediCal provider, the center is committed to enabling participants to age gracefully within their community. The Physical Therapist conducts thorough assessments of program participants' functional capacities and develops tailored treatment plans accordingly. Serving as a key member of the multidisciplinary team, the Physical Therapist provides expert guidance and acts as a consultant to the Program Director. By delivering evidence-based therapeutic interventions and overseeing personalized activities, the Physical Therapist contributes significantly to enhancing social interaction, cognitive function, and physical mobility within a supportive and inclusive environment.


This position is open to an independent contractor or part-time regular employee.


DUTIES AND RESPONSBILITIES

  • Evaluate participants’ physical condition and functional status. Establish goals and set up treatment plans to administer physical therapy techniques as prescribed by the referring physician.
  • Develop and implement the therapy maintenance program and supervises work of Rehabilitation Aides.
  • Provide physical therapy treatment; instruct and demonstrate to families and others the therapy and rehabilitation procedures indicated by the participant’s assessment.
  • Provide general instruction to principles of physical therapy to staff as needed. Provide in-services and concurrent treatment.
  • Record in participant’s chart services given, maintain progress notes and schedule participant for further treatment
  • Work in close cooperation with staff and participate as a member of the multidisciplinary team
  • Meet all administrative requirements such as attending meetings, record keeping, etc.
  • Reevaluate the needs for physical therapy for all participants of the Adult Day Health Care program
  • Must be able to safely perform all physical and manual movements needed for job functions including but not limited to: effectively and safely lifting a 25 pound weight; transferring clients; assisting with personal care; meal service and special events; assisting participants with personal care; escorting clients home; and assisting with loading and unloading client vans
  • Other duties as assigned, including training new therapists and rehab aides, and providing services to staff


Requirements:

MINIMUM REQUIREMENTS

  • Bachelor’s degree in physical therapy
  • Licensed by the Physical Therapy Examining committee of the California’s Medical Board
  • Current CPR and First Aide certificates
  • Two years of experience working with elderly or chronically ill. Experience working in a multidisciplinary team is preferred.
