Jefferson Healthcare is looking for a motivated and caring individual to join our team as a full-time Medical Assistant to focus on Care Gap Closure! We have a tight-knit team that is committed to providing compassionate patient care, including those attributed to the Accountable Care Organization (ACO). The Health Navigator will be responsible for patient outreach to assist with coordinating preventative care visits and screenings. This role will work closely with the primary care and care coordination teams to provide support to our patients and their families while focusing on internal and external quality measures.

What you'll need:
  • High School degree or equivalent required
  • MA or LPN required
  • Previous medical/clinical experience preferred
  • Experience working in a community-based setting for at least 1 to 2 years preferred
  • Community Health Worker formal training such as from a college or other education institution is required OR successful completion of Jefferson Healthcare sponsored training in the first 90 days of employment
  • Understand the community served - community connectedness.
  • Good communication skills, such as listening well, and using language appropriately.
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients

Schedule: 1.0 FTE; 40 hours/week; Day Shift; No Weekends!
