Transitional Care Liason

JOB DESCRIPTION / RESPONSIBILITES

Dynamic is a family owned, Home Health, Hospice, and Caregiving Agency seeking a dedicated Transitional Care Coordinator /Marketing Representative to join our team. The candidate will work to develop and maintain a positive working relationship with established and potential referral sources. Including but not limited to: hospitals, skilled nursing facilities, inpatient rehab facilities, assisted living facilities, and doctor’s offices.

You will meet with hospital case managers, social workers, and physicians to help coordinate care for our new patient referrals and educate patients and the community on our specialty programs and the services we offer.

You will also collaborate with our clinical and administrative team to facilitate a smooth intake process and communicate with new or potential patients and their family members to provide additional resources and ensure a positive experience with our agency.

QUALIFICATIONS

• Caregiving/ Home Health/ Hospice/ Palliative care experience
• Knowledge/experience in Marketing
• Ability to communicate clear and respectfully to patients and families
• Excellent customer service, time management, and organizational skills
• Strong relationship building skills
• Clinical background is a plus!
