Sonoma Specialty Hospital, as the county’s only Long-Term Acute Care Hospital, is dedicated to caring for patients that require an extended acute care stay due to severe illness or catastrophic injury. We accept patients from the ICU, Step-Down and Telemetry units of partner hospitals and continue their care for as long as 45 days. This care focuses on the challenges of getting patients off of ventilator support, healing severe wounds, aiding their recovery following surgical procedures, and providing rehabilitation to speed their return to daily life. Our Hospital provides ICU, Surgical, and Step-Down care with dialysis, multi-specialty therapies and ongoing planning for the patient’s transition back home or into a lower level of care. Job Summary
The purpose of the RNA is to interact with the patient and provide skill practice in such activities as walking and mobility, dressing, and grooming, eating and swallowing, transferring, amputation care, and communication in order to improve and maintain function in physical abilities, and ADLs and prevent further impairment.
Education and/or Experience
- High School Graduate
- Completion of RNP Certification Course required.
- Current California Certified Nursing Assistant (CNA) required.
- Current BLS required.
- Minimum one year experience as a CNA in long-term care preferred.
Recommended responsibilities (include, but not limited to the following):
- Administer restorative activities specific to patient’s needs.
- Perform transfers, bed mobility, positioning and range of motion (passive/active).
- Perform general strengthening exercises.
- Ensure the placement of restorative devices/equipment (e.g., splints, positioning devices).
- Assist with turning and positioning of patients as needed.
- Document per treatment activity and summarize progress for patients.
- Report and chart significant changes in patient condition, evidence of pain or motivational
- Notify DOR, Charge Nurse or appropriate therapy discipline of problems, referrals and
reassessment needs. - Follow appropriate safety, hygiene and infection control measures.
- Ensure adequate restorative supplies to meet the needs of the patients.
- Maintain consistent patient care schedule and be efficient in use of time.
- Coordinate restorative treatments with other facility departments.
- Attend all meetings and in-services as required.
- Follow policies and procedures.
- Record data, documentation and report to QAPI.
- Identifies potential problems/issues and reports to DOR in a timely manner.
- Able to participate in physical activities that may require bending, squatting, reaching, twisting,
repetitive grasping, frequent pushing and pulling and frequent lifting up to 150 pounds with

- Computer skills required. Basic math knowledge, including addition, subtraction and multiplication.
