LCPC / LCSW-C - Direct Hire – Wilmington Delaware

Our client in Wilmington Delaware is seeking an LCPC / LCSW-C (Licensed Clinical Professional Counselor/Social Worker) to join their team in a Direct Hire position.

The purpose of this position is to provide responsive, client centered individual, group and family therapy to children, adolescents, adults and their families. Therapists will be responsible for developing/implementing Individual Treatment and Transition/Discharge Plans; maintaining current and accurate case documentation; coordinating service provision in collaboration with internal and external members of the care team; conducting clinical assessments and completing activity and performance reports.

Essential Functions:

  • Provide individual, group, and family therapy, within the scope of professional license, to children, adolescents, and their families.
  • Learn and implement evidence-based practices selected by the organization, which may include attending on- or off-site training, participating in ongoing supervision, consultation, and/or continuing education, committing to delivery of the practice as expected, following practice guidelines with fidelity, and engaging in fidelity monitoring.
  • Provide effective management of assigned cases which includes but is not limited to: developing, monitoring, and reviewing Individual Treatment Plans, Safety Plans, and Transition/Discharge Plans, and collaborating in transition/discharge planning.
  • Serve on the Interdisciplinary Care Team for each youth served, to include consultation with the providing psychiatrist regarding client needs and interventions, and working collaboratively with Case Managers, Unit Supervisors, and other program staff under the guidance of the Care Team Coordinator.
  • Evaluate the child’s/youth’s/family’s progress towards goal attainment on a designated basis in line with individual treatment and transition/discharge plan goals that are individualized, have strengths and service needs clearly identified, and are developed in behaviorally measurable terms.
  • Complete psycho-social intake assessments and ongoing clinical and functional assessments that include participation by child/youth and parents/guardians.
  • Provide clinical services that are trauma-informed and aligned with purpose and values.
  • Coordinate with the Board of Child Care staff members and community resources to address clients’ service needs.
  • Maintain effective working relationships with families, children/youth, colleagues, consultants and community agencies.
  • Create Guidance Plans for the living environment with input from the care team and act as a consult to the care team, family, and school regarding clinical and behavioral presentations and interventions.
  • Use Guidance Plans to create 1-1 supervision forms, as needed, for direct care staff and monitor staff in the delivery of those services.
  • In collaboration with the case management department, the therapist shall facilitate family focused skill building groups on a monthly basis, ensuring that there is proper documentation of these groups.
  • Ensure at least once per week face to face individual therapy session with each youth assigned to case load. As appropriate, family sessions shall be scheduled at a frequency determined between the therapist, youth, family and clinical supervisor.
  • Ensure weekly group educational/clinical sessions occur with the assigned caseload.
  • Complete required documentation and reports within the defined time lines.
  • Ensure assigned case records meet documentation standards.
  • Engage in the peer review process.
  • Record and submit billing data within the defined time lines. Complete necessary financial and programmatic reporting as may be required by grants and/or alternative funding source requirements.
  • Promote child/youth/family understanding of available Board of Child Care and community resources and facilitate referrals as appropriate.
  • Participate in biweekly clinical team meetings, weekly care team meetings, departmental and program-specific interdisciplinary team meetings, as well as ITP and IEP meetings as appropriate.
  • Act as Therapist on Call on a rotating basis or maintain a schedule that includes a weekend shift to provide family engagement activities.

Work Environment:

  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
  • In the nature of being at a residential and school environment there may be times you experience conditions in which challenging behavior can be exhibited from our youth to include but not limited to, physical agitation, physical violence and/or emotion outburst.
  • Physically intervene and restrain student, as necessary. Provide crisis intervention.

Position Type/Expected Hours of Work:

This is a full-time position. Candidates must be willing to work flexible hours. Standard days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m., and must include two evening shifts (e.g. 11:30 a.m. – 8 p.m.). Must rotate through as Therapist on Call. Attendance at occasional weekend or evening family engagement events may be expected. Eligible for flex schedule. Non-essential for snow emergencies.

Travel:

May include some travel by car to community-based meetings, to family homes to provide therapy, to the local hospital emergency department to support a youth who is awaiting psychiatric evaluation following an emergency petition, and/or transportation of youth to community-based meetings. Travel is primarily local, although some out-of-area, occasional out of state travel, and overnight travel may be expected.

Required Education and Experience:

Master’s Degree in Social Work, Counseling, or a related field from an accredited college or University

Current LMSW, LGPC, LCSW-C, LCPC, LGMFT, LCMFT, LGPAT, or LCPAT Maryland State License

Demonstrated experience working with children, adolescents, adults and families with acute mental health and behavioral health challenges.

Preferred Education and Experience:

Experience providing therapeutic services to children and families

At least one-year post-graduate experience

Current LCSW or LCSW-C Delaware State License

Certification as a Certified Clinical Trauma Professional (CCTP), or equivalent

  • Knowledge of normal and abnormal human growth and development
  • Ability to assess family strengths, level of functioning, and service needs
  • Ability to assess children’s and youth’s strengths, levels of functioning, and service needs
  • Ability to effectively develop and implement service treatment plans which include transition/discharge planning
  • Ability to analyze case information and reach a sound decision based on objective information
  • Ability to render a diagnostic impression using the DSM-5
  • Ability to develop/implement effective treatment strategies and interventions
  • Ability to provide individual, group and family verbal therapies using evidence-based protocols
  • Ability to work cooperatively with others in a treatment team
  • Ability to effectively express ideas clearly and concisely, orally and in writing
  • Ability to utilize electronic health record system.

Successful completion of IATP requirements for a Certified Clinical Trauma Professional within 6 months of hire (may be waived with proof of equivalent certification at time of hire).
