LPN Licensed Practical Nurse

Position Summary:

The LPN Visit Nurse will provide skilled nursing care to homebound clients, delivering compassionate and personalized care in their homes. They will conduct visits to assess client needs, administer treatments, and educate clients and their families on health management.

Responsibilities:

• Conduct home visits to follow up on client needs, implement care plans, and deliver skilled nursing care.
• Administer medications, treatments, and therapies as prescribed by the physician.
• Perform comprehensive assessments and document client progress, including vital signs, medication administration, and wound care.
• Educate clients and their families on health management, medication adherence, and disease prevention.
• Collaborate with healthcare professionals, including RN case managers, physicians, therapists, and social workers, to ensure coordinated care.
• Communicate effectively with clients, families, and caregivers to address concerns and provide support.
• Maintain accurate and up-to-date client records in accordance with regulatory standards and agency policies.
• Participate in ongoing training and professional development activities to enhance clinical skills and knowledge.
• Adhere to all federal, state, and agency regulations and policies governing home health care services.

Qualifications:

• Current LPN licensure in the state of Minnesota.
• Experience with home care preferred, but willing to train serious candidates.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and interpersonal skills.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Reliable transportation and a valid driver's license.
• Basic computer skills for electronic documentation and communication.
• CPR certification required.
• 9Must be able to pass background checks and drug screening.

Schedule:

Monday through Friday, no evenings or weekends, occasional on-call rotation every 5 to 6 weeks.
