PRIMARY FUNCTION/RESPONSIBILTY

Responsible for case management functions that include direct services to foster families and youth, maintenance of records to keep in compliance with licensing regulations, and consultation with foster families and agencies to ensure that the programs meet the needs of the children in residence.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Administrative
  • Present and participate in regular staff meetings.
  • Assist Licensed Administrator with training duties, to include development, preparation, and delivery of training.
  • Maintain and monitor case files, progress logs, educational portfolios, and incident reports weekly for completeness, accuracy, compliance, and security.
  • Responsible for preparation of treatment plans and scheduling treatment plan meetings.
  • Assist Licensed Administrator with consistent interpretation and enforcement of Policies and Procedures.
  • Ensure compliance with all licensing regulations, contract, and purchased placement requirements.
  • Prepare youth records for audits as conducted for state licensing to ensure annual requirements are being met.
  • Provide direction and support to foster families to ensure compliance with social programs and services.
Program
  • Recruit and interview potential foster/adoptive parents.
  • Evaluate and determine appropriate child placements.
  • Process intake and admission documents for youth.
  • Travel to foster homes and conduct a minimum of once monthly home inspections following SJRC guidelines and state licensing standards. Obtain all necessary documentation from foster home as needed.
  • Monitor and verify children’s medical/mental health/education needs are being met by communicating with foster families, monitoring all health visits, participating in educational meetings, and obtaining all documentation for youth file.
  • Attend court and State meetings with, or as a representative of, the foster family.
  • Prepare discharge plans and ensure smooth transition of services.
  • Purge files when youth are discharged or foster homes close, and ensure records are stored appropriately.
  • Prepare outcome reports, youth roster, and youth documentation to meet state licensing requirements.
  • Identify, develop, and maintain positive and professional relationships with support services (e.g., Child Protective Services, CASA workers, therapists, case workers, doctors, law enforcement, juvenile court personnel, school officials and community agencies).
  • Traveling within the region 8 (San Antonio and 27 counties surrounding)
  • Attend community events as a representative of SJRC.
  • Available on call 24 hours/day, 7 days a week.
General
  • Perform other related duties as required.
QUALIFICATIONS

Required:
  • Bachelor’s degree from an accredited college or university in social work or other human services field.
  • 2 years’ experience conducting child-placement activities.
Preferred:
  • Master’s Degree from an accredited college or university in social work or other human services field.
Certifications/Licenses/Registrations:
  • Valid Texas Driver’s License, good driving record, ability to provide own transportation and proof of current automobile insurance.
  • CPR/First Aid Certification (provided).
  • SAMA Certification (provided).
  • Age minimum of 21 years of age per licensing standards.
PHYSICAL/MENTAL REQUIREMENTS

Physical Demands
  • Body mobility to stoop, kneel, bend, reach, and walk and walk briskly in order to interact with and monitor children.
  • Stamina to work long days and drive long distances, travel of 70%.
  • Moderate to heavy lifting to up to 60 pounds.
Visual and Cognitive Demands
  • Must have the ability to communicate both verbally and in writing.
  • Must have the ability to give and receive verbal and written instructions (with or without hearing aids).
  • Must have the ability to read fine print, have sustained vision, and peripheral vision (with or without glasses).
Environmental Demands
  • Environment requires dexterity to remain calm and adapt to fast paced and emotional environments consisting of children with moderate to severe emotional and psychological needs.
Work Environment
  • Home-like settings with varying moderate to high degrees of background noise.
  • Light and ventilation are found in a typical home setting.
  • Travel of 70% with varying moderate to high degrees of road noise.
  • Varying air conditions due to weather, allergens, and car exhaust in vehicles with open ventilation (windows down).
  • Campuses are smoke-free, except on permitted campuses in designated areas.
Possible Hazards
  • Possible injury when lifting or moving (child or objects) if employee fails to use proper body mechanics, or if proper techniques are not used when the use of SAMA containment techniques are required.
