Behavioral Case Manager

BEHAVIORAL CASE MANAGER JOB SUMMARY:

• Facilitate appropriate healthcare outcomes by providing care coordination support and education for members through the use of care management tools and resources.
• Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues
• Educated/empowers customers to ensure compliance satisfaction and promote patient advocacy. Optimize total costs.

BEHAVIORAL CASE MANAGER HIGHLIGHTS:

• Work schedule: Mon-Fri, 8a-5p
• HYBRID opportunity working for a major health plan: 50% Remote / 50% Field visits
• Candidate must live in Johnson County, Shawnee County, OR Sedgwick County to be considered
• Benefits are available to full-time employees after 90 days of employment; 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.
• The compensation for this role is $29.45 per hour

BEHAVIORAL CASE MANAGER REQUIREMENTS:

• Bachelor Degree in social work, human services, behavioral health, public health or related field required
• Case management experience required; Long term care experience preferred.
• Home health experience preferred.
• Proficient in Microsoft office including Excel competent.
• Ability to work independently from home.
