Description

The Clinical Registered Nurse at Mendocino Community Health Centers (MCHC) provides general nursing care to patients in the clinic and over the phone. The RN plays an essential role in providing high quality patient care and access through visits using protocols and transitioning care to a provider whenever able or needed. Within the Care Team, the RN works as a point person for daily functions and triage.

Essential Duties and Responsibilities Include:

• Provides organized nursing care for individual patients or assigned patient groups, adhering to established care standards and the nursing process. Directs and supervises nursing staff at different levels, ensuring their activities align with patient care goals. Coordinates care across disciplines, employing critical thinking, professional judgment, and independent decision-making to optimize patient outcomes.
• Conducts phone triage assessments and/or in-person triage for patients with urgent medical needs, determining the necessary level of care and intervention. Ensures accurate documentation of triage calls, critical values, verbal orders, and provides comprehensive reports to hospital staff to maintain patient continuity of care.
• Gathers pertinent data related to the patient's health or situation and analyzes it to identify diagnoses and care needs. Works with Care Team to develops a comprehensive plan aimed at achieving desired outcomes. Evaluates progress toward desired outcomes and adjusts the plan as needed.
• Ensure follow-up for all patient ED visits, organize care for discharged hospital patients, collaborate with the care team, assess if additional follow-up is needed, and meticulously document all information in the patient's chart.
• Collaborates closely with the Population Health team to bolster chronic care management efforts by identifying, contacting, and scheduling patients who are overdue for screening exams, diagnostic tests, immunizations, or other health maintenance tasks.
• Provides vaccine and injection verification to the Care Team as needed for safe and accurate administration.
• Acts as a specialist in nursing procedures, supporting the Nursing Leadership Team in the effective training and onboarding of staff. Additionally, contributes to educational efforts as required.
• Assists in the review, development, and implementation of clinical policies and procedures when needed.
• Follows Universal Precautions, OSHA standards and is familiar with reporting adverse conditions including injuries and occupational exposure.

• Valid State of California Certificate of Registration as a Registered Nurse
• Satisfactory completion of a prescribed course of instruction in intravenous therapy, approved by California
• Maintain current BLS certificate.

• Two or more years' experience, as an RN, in an outpatient setting
• Bilingual, English/Spanish, fluent
