PLEASE CONFIRM W/ THE CANDIDATE - AWARE OF AND COMMITTED TO o ABILITY TO BE ON FEET ALL SHIFT AND MOVE o OPEN TO THE POSSIBILITY TEMP TO PERM o THERE IS LITTLE TO NO SCHEDULE FLEXIBILITY IN SCHEDULING PER REQUISITE SHIFT o THEY ARE NOT A CHOP FTE, THEY WILL BE A CONTRACTOR o THEY'LL NEED TO SUBMIT TIMESHEETS WEEKLY AS WELL AS PUNCH IN EACH SHIFT o THEY'LL NEED TO FOLLOW CHOP AND DEPT POLICY o SCHEDULE AND TRAINING DETAILS BELOW SCHEDULE NOTES o DAYS/NIGHTS, ROTATING SHIFTS. EVERY OTHER WEEKEND AND HOLIDAY COMMITMENT. o ANYONE COULD BE SCHEDULED TO WORK ANY SHIFT ANY TIME. THOUGH THE MANAGERS WILL DO THEIR BEST TO PROVIDE FOR CONSISTENCY, THIS ROLE REQUIRES VARIABLE SHIFTS INCLUDING HOLIDAYS, WEEKDAYS, AND WEEKENDS (DAY AND NIGHT SHIFTS). WORKERS WILL BE PROVIDED WITH SCHEDULES BIWEEKLY. TRAINING FOR THE FIRST 3 DAYS IS MANDATORY AND FULLTIME (8:30-5). AFTER ORIENTATION, ~ 90 DAYS, WORKERS WILL SHADOW ACROSS ALL SHIFTS. THERE IS NO FLEXIBILITY IN THIS REQUIREMENT. o 2 LOCATIONS @ MAIN HOSPITAL o FT & PT ROLES PT MIN OF 20 HRS/ WK 1ST 3 MONTHS ARE FT THEN PIVOT TO THE PT o SHIFTS & HOURS ANYONE COULD WORK ANY SHIFT VARIABLE WEEKDAY + WEEKEND 2 WEEK LEAD TIME W/ SHIFTS o 4 SHIFTS INCLUDE: WORKERS DO NOT CHOOSE SHIFTS 8:30AM 5:00PM 9:00AM 5:30PM 10:30AM 7:00PM 3:00PM 11:30PM

Provides nutrition care services for patients/clients to address prevention and treatment of acute and chronic diseases and conditions and the promotion of overall health and wellness in conjunction with the registered dietitian nutritionist. Participates in enteral feeding preparation by adhering to aseptic technique when producing specialty formula and human milk mixtures for patient consumption. Adheres to standards of food safety and sanitation, best practices for formula and human milk preparation in a hospital setting, regulatory standards, departmental and hospital policies and procedures in the execution of assigned duties. Essential Functions

Assists in the development of educational materials and other projects.

Provides education with regard to nutrition and lactation.

Participates in site specific education events as requested

Maintains friendly, efficient demeanor and models CHOP ICARE Values and Service Standards.

Participates in ongoing quality assurance and performance improvement activities as requested.

Maintains productivity, screening and other documentation of activities as requested.

Product inventory and management.

Participates in training other employees as requested.

Maintains confidentiality of patient information.

Ability to communicate effectively verbally and in writing to patients and families, public, medical staff and physicians.

Must be able to read English with understanding and write English legibly.

Basic knowledge and application of medical nutrition therapy in pediatric patients.

Basic knowledge of nutrient analysis, word processing, and spreadsheet software.

Competence in nutritional content, sources and use of commercial supplements, tube feedings, human milk, infant formulas and feeding

modulars (fat, protein, carbohydrate, vitamins and minerals).

Ability to perform increasingly complex mathematical calculations such as fractions, percentages, ratios, proportions and metric measures.

Computer proficiency is required.

Prior work experience in a hospital setting or WIC program Preferred

Licenses, Certifications, and Registrations

Nutrition and Dietetic Technician-Registered (NDTR) Commission on Dietetic Registration within 6 months Required

Completes nutrition assessments of patients within scope of practice of Dietetic Technician-Registered based on screening and documentation guidelines

Nutrition and Dietetic Technician-Registered (NDTR) and Serv Safe Certification
