Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We are currently seeking a Case Manager with our Be Connected team out of Sierra Vista.

General Description: The Be Connected case manager is a community-based position. The Be Connected case manager outreaches service members, veterans, and their families in the community to educate them about services and supports available to them in their area. They then assist those in need by connecting them to necessary services and supports, following a social needs assessment. In addition to educating service members, veterans, and their families on available services and programs, the Be Connected case manager may conduct trainings and participate in outreach activities for both military and community-based events.

The Be Connected case manager will initially be responsible for the coordination of care activities, follow up activities, resource development and staffing cases with clinical team members, until the client is successfully connected to ongoing services.  The Be Connected case manager will also coordinate with medical, community-based services, and the military support community to ensure the client is meeting service plan goals through service coordination. The Be Connected case manager may also:

  1. Monitor clients and their daily activities, and coordinate care within the Be Connected and CBI program to ensure program compliance.
  2. Communicate with the both Be Connected and CBI clinical team to ensure client’s service plan goals and objectives are clear. Perform case management duties as defined in each client’s individualized service plan.
  3. Staff cases as needed in supervision activities, or other clinical meetings offered by both the Be Connected and CBI program.  
  4. Schedule, coordinate, facilitate, and deliver trainings and presentations at various locations and times when requested.
  5. Represent and promote the Be Connected Program in the community at outreach events with stakeholders and community members.
  6. Consistently and reliably attend mandatory meetings with the Be Connected team, as well as any and all mandatory trainings, supervisions, and meetings as requested by CBI.
  7. Provides in field crisis and other related supportive services, until the client can be connected to the appropriate resources to manage the need.
  8.  Teaches skill building activities to the client as needed. 
  9. Documents interaction with each client prior to the end of his or her scheduled shift in specified database (e.g. NextGen, HMIS, or other).
  10. Responsible for shift end staffing report and any other ad-hoc reports needed.
  11. Ensures each client is receiving quality of care in all areas of support.
  12. All other duties as assigned.  

Minimum Education & Experience Required:

  • Bachelor’s degree in a field related to behavioral health preferred OR a combination of education, training, and/or experience
  • 1 year experience in a behavioral health position providing outreach and engagement activities
  • Valid Arizona Driver’s License with 39 month clear MVR
  • Behavioral Health Technician (BHT)
  • CPR/First Aid
  • CPI
  • Arizona Fingerprint Clearance card, site specific

 

Work Environment/Conditions and Physical Requirements:

  • Able to move around quickly and freely in response to crisis situations
  • Must be able to move and/or lift a minimum of 150 pounds
  • Able to drive to multiple locations upon notice or need without restriction.
