Job Description

Administrative support for the clinical team, so that goals of the agency can be achieved.

Home health care experience preferred
oversees the coordination of scheduling activities
supports documentation review process for discharge, inpatient transfer, re-certification, and start of care
follow up with clinical as needed regarding corrections
Supports Intake and liaison process as needed
Attends staff conferences as directed by the Agency Administrator
Attends in-service and clinical staff meetings as scheduled
reports deficits in documentation standards to Team Leaders, Q/A and Agency Administrator as needed
Able to complete home visits
