Family Caregiver Support Program is a service available to unpaid caregivers of adults who need care in their homes. Staff provide practical information and connect the caregiver to local free or low-cost resources/services meeting their needs.

In this position, you will provide case management services to unpaid family caregivers, including information and coordination of care. Provide transformative services to individuals meeting the nursing facility level of care. Utilize a variety of assessments and screening tools to develop care plans. Seek to obtain respite and supplemental services to enrolled unpaid/family caregivers. Provide information and referral to community resources as well as long-term services and supports that best fit client needs. Facilitate caregiver support groups and caregiver classes. As appropriate, link caregivers to mental health services and other services deemed relevant and necessary. Engage in events to outreach to community partners.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Exhibit appropriate interpersonal skills and engagement strategies to complete screenings and assessments in a timely manner;

  • Present a variety of State caregiving programs at various public forums;

  • Complete timely and accurate Care Plans while ensuring the appropriate services are being offered to clients;

  • Request authorization for client services accurately and timely;

  • Facilitate caregiver support groups and educational classes;

  • Promote the program by participating in outreach activities;

  • Provide clients with information to assist in decision-making and problem-solving related to their caregiver roles;

  • Advocate for clients when appropriate;

  • Complete all documentation and entries into State databases within the required timeframe set forth by State standards;

  • Maintain professional acumen when interacting with clients, staff, agency partners, and leadership;

  • Complete all other duties as assigned by the branch supervisor and/or director.

QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:

Staff administering the full TCARE assessment/consultation and service planning must meet the minimum qualifications of an AAA Case Manager and certification in the State of Washington. These qualifications include the following minimum education and experience requirements:

  • A Master's degree in behavioral or health sciences and one year of paid on-the-job social service experiences; or

  • A Bachelor's degree in behavioral or health sciences and two years of paid on-the-job social service experience; or

  • A Bachelor's degree and four years of paid on-the-job social service experience.

  • Must apply for Agency Affiliation within 30 days of hire.

  • Must have reliable transportation, insurance and an active driver's license and be willing to use personal vehicle in the course of work.

KNOWLEDGE, SKILLS, & ABILITIES:

  • Knowledge of elder care and disabled adult caregiving issues;

  • Ability to perform face-to-face assessments and screenings;

  • Proficiency with Microsoft Office suite and experience with database systems;

  • Experience with leading large groups and public speaking.

PAY RANGE (Hourly/ Non-Exempt): $25.89- $31.53

*All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $25.89 per hour.
