Job description

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated RN Case Manager to work with us in Pinellas County while providing exceptional care on the following schedule: Monday-Friday 8a-5p with occasional weekend day, 1 summer and 1 winter holiday requirement. Coverage territory Pinellas county working in both home/facility team.

What you'll Do:
  • Reviews and updates the care plan, physician orders, pharmacy profile and other records.
  • Educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.

What You'll Need:
  • Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
  • Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
  • Valid Florida State Driver's License This is only required if colleague chooses driving herself/himself as mode of transportation when reassigned to work at another facility. This is not a requirement if the colleague chooses an alternative means of transportation when reassigned to another facility based on business needs. Regardless of the mode chosen, both are at the colleague's own expense, with the exception of mileage reimbursement eligibility when traveling between facilities while on company time.
